Government Recognition Initiative & Projects (GRIP)
NCQA GRIP manages government contracts that promote the Patient-Centered Medical Home (PCMH) model in Federally-Qualified Health Centers (FQHCs). Current contracts with the Health Resources and Services Administration (HRSA) provides financial support for the costs of NCQA PCMH Recognition and technical assistance in achieving Recognition and Massachusetts Health Policy Commission (HPC) Recognition.
MA HPC PCMH PRIME
MA HPC PCMH PRIME Massachusetts’ Health Policy Commission (HPC), in partnership with NCQA, has developed the MA HPC PCMH PRIME certification program. This voluntary certification program, available to Massachusetts practices, will endorse practices that meet NCQA's rigorous guidelines for patient-centered care and succeed in maintaining additional behavioral health care services for their population. Apply for HRSA’s PCMHHI Program
You may now notify HRSA of your intent to participate in the APCMH Initiative by submitting a Notice of Intent (NOI)
in the HRSA EHB. Go to the Grant Folder and click the ‘HRSA Accreditation/PCMH Initiative’ link. For assistance with your EHB account please contact the BPHC Helpline: (877) 974-2742. For assistance in completing an NOI through the EHB, please refer to the HRSA EHB NOI training presentation. Please be advised that any NOI submitted through the old NOI process after September 30th, will not been processed. Please make sure that your NOI has been submitted through EHB. However, if an NOI was submitted prior to Sep 30th, please do not submit a duplicate request through EHB.
