Survey Tool for the Certification of Physician Organizations
The Survey Tool includes all information needed to prepare and submit materials for an NCQA survey. Organizations applying for certification must complete the Survey Tool and submit it to NCQA to begin the survey process.

For web-based products, NCQA does not make any commitments with respect to the use or performance of the Software and Services with browsers other than Netscape 6.0 or higher, or MS Internet Explorer 5.5 or higher. Please review the license agreement for web-based tools prior to purchase.
