Congratulations on your decision to become NCQA Accredited or Certified. The following information will provide information on how to apply for and schedule surveys for the following NCQA products:
Applications and Pricing
Click the link below to go to the NCQA Publications and Products to request an application packet for any program. Application packets contain the application; agreement (contract) and pricing (see Exhibit A of the agreement); information to familiarize you with NCQA’s application process and instructions on how to complete and submit the application.

Once you have reviewed the application packet, you may want to refer to the Applications and Scheduling Frequently Asked Questions (FAQ) for answers to general questions.
Application and Scheduling Clarifications
If you have additional questions, click on the Applications and Scheduling Policy Clarification Support (PCS) link below to enter and submit your question to NCQA’s Application and Scheduling staff. PCS allows NCQA staff to receive, track and respond to your inquiry in a timely manner.
Once you click on the link below, enter your contact information at the top of the screen and select Applications/ Scheduling by clicking the drop-down box next to Product Type. Scroll to the bottom of the list and select Other for both the Standard Category/HEDIS Domain and the Standard Measure. Enter your question and click Submit Question at the bottom of your screen.
