About the Program

Intro.jpgThis seminar is designed for those individuals struggling with the challenges and issues facing disease management and complex case management programs. Faculty will help participants understand the complex case management, disease management and continuity of care standards (QI 7, 8 and element A of 10), as well as discuss the distinction between and the interrelationship among the disease management, case management and continuity and coordination of care standards.

Additionally, the requirements surrounding delegation to a disease management organization will be discussed.

The seminar is based on 2008 standards for Health Plans.

Registration is from 8:00 a.m. – 8:30 a.m. Seminar hours are 8:30 a.m. – 4:30 p.m.  Continental breakfast and lunch will be served.
