Patient-Centered Medical Home Resources

NCQA's Physician Practice Connections®- Patient-Centered Medical Home™ (PPC-PCMH) program reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics and American Osteopathic Association and others in the revision of Physician Practice Connections® to assess whether physician practices are functioning as medical homes. Building on the joint principles developed by the primary care specialty societies, the PPC-PCMH standards emphasize the use of systematic, patient-centered, coordinated care management processes.

Patients Find Medical Home in Pennsylvania: Apr. 2009 (ACP Internist)
-- A group of physicians in southeastern Pennsylvania are among the lucky and proactive few practicing in some of the first payer-supported patient-centered medical homes (PCMH) in the country. Led by Gov. Ed Rendell’s Chronic Care Commission, the first phase of the Pennsylvania project includes 32 internal medicine and family practices and six major payers.

New Orleans-Area Clinics Receive Medical Home Certification: Mar. 2009 (Louisiana Medical News)
-- The effort to establish "medical homes" for patients in the New Orleans area reached a major milestone with the National Committee for Quality Assurance's recognition of 37 clinics for that achievement.
