PCMH Congress 2015

As a professional dedicated to the Patient-Centered Medical Home, NCQA would like to invite you to submit an abstract to present at this first annual meeting devoted entirely to transforming care through the medical home model of care. Tracks will also include information on preparing for, achieving, and maintaining NCQA PCMH Recognition.
PCMH Congress 2015 features a robust 3-day agenda of hands-on education for attendees at all stages of the PCMH transformation. NCQA PCMH Certified Content Experts will also have the opportunity to earn credits towards maintenance of certification.
This official NCQA national conference will take place at The Hilton San Francisco Union Square, San Francisco, CA from October 9-11, 2015. Located in the heart of downtown San Francisco—and only blocks from the city’s public transit system—the Hilton Union Square offers sweeping views of the city and easy access to Fisherman’s Wharf, Chinatown and Nob Hill.
Those selected to present at PCMH Congress 2015 will have the unique opportunity to present a 1-hour session in front of up to 1000 conference attendees. Presenters also receive complimentary registration (a $1500 value), and the opportunity to earn approximately 20 CME credits.
Our Call for Abstracts is now open. We encourage you to share your PCMH expertise and experiences as an Expert Speaker by submitting an abstract pertinent to furthering the transformation of primary care practices into PCMH sites.
Your comprehensive session abstract proposal(s) MUST be submitted by Saturday, January 31, 2015 to be considered for inclusion in this year’s agenda. We are seeking proposals for presentations that include, among others:
- Best practices in PCMH achievement and/or maintenance
- Financial aspects of PCMH
- Building a transformative team within your practice
- Clinical “Case Studies” or Successful PCMH journeys
- ACO and meaningful use
If you have demonstrated expertise and possess an interest and passion in a given area/topic, then we invite you to submit through the Comprehensive Session Call for Abstracts web page to be considered as a speaker at the 2015 PCMH Congress.Your proposal may also be considered for additional NCQA educational opportunities in 2015.
