Diabetes Recognition Program: Grant Funding Available

NCQA’s Diabetes Recognition Program recognizes clinicians who use evidence-based measures to provide quality care to patients with diabetes. With support from the Helmsley Charitable Trust, we’ve recently refreshed the program to include an updated platform, an updated measure set and ongoing Recognition status. The refreshed program offers:
- Flexibility. Practices can submit data through FHIR® or a CSV file.
- Personalized service. Clinicians have more interaction with NCQA, and are assigned an NCQA representative who works with them throughout the Recognition process as a consistent point of contact.
- User-friendly approach. Requirements remain meaningful, but with simplified reporting and less paperwork.
- Continuous improvement. By reviewing your progress more often, you can keep performance improvement at the top of your priority list.
- Alignment with changes in health care. The program aligns with current public and private initiatives, and can adapt to future changes.
Grant Funding Available
For a limited time, practices can request grant funding through the Helmsley Foundation to cover application fees and standards for up to five clinicians. Use the Q-PASS sponsor code DPPS24 during the application process to be considered for grant funding.
