About the On-Demand Training
NCQA’s PCMH Recognition program has evolved to feature a set of six concepts that make up a medical home. Underlying these concepts are criteria (activities for which a practice must demonstrate adequate performance to obtain NCQA PCMH Recognition) developed from evidence-based guidelines and best practices.
Each concept’s criteria outline the features of PCMH transformation and how NCQA evaluates a practice’s ability to function as a patient-centered medical home. You will review an overview and description for each standard, as well as criteria examples, to help you in the transformation process.
This self-paced training focuses on the six PCMH concepts listed below:
1. Team-Based Care and Practice Organization (TC)
2. Knowing and Managing Your Patients (KM)
3. Patient-Centered Access and Continuity (AC)
4. Care Management and Support (CM)
5. Care Coordination and Care Transitions (CC)
6. Performance Measurement and Quality Improvement (QI)
