PCMH Recognition Annual Reporting: Continued Success
This education program is designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission under the Patient-Centered Medical Home program. Expectations for those transitioning from the 2014 PCMH recognition as well as those that have achieved Recognition under the current PCMH requirements will be discussed in detail.
Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that the practice is already collecting internally and for other sources (e.g. state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

- Explain how Annual Reporting supports the maintenance of the medical home model and a culture of continuous quality improvement within a PCMH practice.
- Describe the Annual Reporting process, including timelines, submission and audit.
