Use of these tables constitutes acceptance of our terms of use as set forth in this usage agreement (the “Agreement”).
SIR Final Table Usage Agreement
This table is made available to assist with calculation of the Standardized Healthcare-Associated Infection Ratio (HAI) HEDIS® measure.
You may not use the HAI SIR Final Table for any other purpose, including, but not limited to, a commercial or research purpose, unless you obtain prior written approval from NCQA and agree to the terms of a license, at NCQA’s sole discretion.
You may not copy electronically or otherwise transmit, redistribute, republish, modify, adapt, translate, reverse engineer, decompile, disassemble, sublicense or otherwise provide the HAI SIR Final Table to any third party. You may not use or include the HAI SIR Final Table in any other print or electronic publication service or product.
This Agreement shall become effective immediately upon your access of the HAI SIR Final Table.
YOU CONFIRM YOUR AGREEMENT TO VOLUNTARILY USE THE HAI SIR FINAL TABLE SUBJECT TO THE TERMS AND CONDITIONS OF THIS AGREEMENT BY USING THE HAI SIR FINAL TABLE FOR HEDIS MEASURE CALCULATION.
IF YOU DO NOT AGREE TO THE TERMS STATED ABOVE YOU ARE PROHIBITED FROM USING THE NCQA HAI SIR FINAL TABLE.
