WASHINGTON, DC—The National Committee for Quality Assurance (NCQA) is releasing a free guide to help organizations working to meet Long-Term Services and Supports (LTSS) standards. A Roadmap to Success in LTSS guides organizations toward meeting NCQA standards to earn accreditation or distinction for case management of LTSS.
With support from The SCAN Foundation and The John A. Hartford Foundation, and with contributions from NCQA’s LTSS Learning Collaborative and Advisory Committee, NCQA compiled a set of practical tools, examples and links to valuable resources to help organizations implement and successfully demonstrate NCQA’s LTSS standards. These standards assess patient-centered care, care planning and care transitions and ensure organizations provide the best possible care.
“NCQA has a long history of developing products and programs to meet identified needs in health care,” said Margaret E. O’Kane, NCQA President. “And there is increasing emphasis on caring for people where they live. We are proud to partner with The SCAN Foundation and The John A. Hartford Foundation to help organizations that provide care to our most vulnerable populations—to ensure that they deliver quality care that coordinates with all of an individual’s health care and supportive service providers.”
The goal for A Roadmap to Success is to make the accreditation process easy to navigate and to help organizations interpret and implement the standards.
NCQA’s Case Management for LTSS Accreditation program and the LTSS Distinction module create an opportunity for community-based organizations, health plans, case management organizations and managed behavioral healthcare organizations to demonstrate their ability to deliver LTSS and coordinate with other health care and service providers.
