Patient-Centered Connected Care: 2020 Pricing

Overview

The Standard Survey price is the fee for eligible providers undergoing the recognition process for the first time and at the time of renewal. Pricing is based on the number of sites brought forward for each recognizable entity:

• $1,550 per site for the first 50 sites.
• $770 per site for sites 51–500 sites.
• $250 for sites 501+.

A nonrefundable application fee (10% of the total survey costs) is due at the time of application for each recognizable entity.
