For Home And Community-Based Service Organizations
Accreditation of Case Management for LTSS is for community-based organizations that coordinate LTSS, including:
- Area Agencies on Aging
- Centers for Independent Living
- Adult foster care
- Other home and community-based organizations
For Health Plans Accredited by NCQA for Health Plan Accreditation
Distinction in LTSS is for organizations that provide managed health services and coordinate social services for LTSS.
- Health plans already accredited by NCQA
- Health plans about to get accredited by NCQA
For LTSS-Only Health Plans or Health Plans Not Accredited by NCQA for Health Plan Accreditation
NCQA Accreditation of Case Management for LTSS for organizations that coordinate LTSS and do not provide medical/behavioral services.
