- NCQA will offer continuing medical education credits and PCMH CCE credits as part of the regular on-demand training.

Introduction to PCMH: Foundational Concepts of the Medical Home An in-depth guide to NCQA Recognition requirements and the process for achieving PCMH Recognition. Standards emphasize practicewide commitment to sustaining PCMH transformation, demonstrating patient-centered care and population health management, care coordination and ongoing quality improvement. Practices are rewarded for integrating behavioral healthcare and addressing the impact of social determinants on patient needs. NCQA requirements are positioned to qualify for the Merit-Based Incentive Performance System (MIPS) Clinical Practice Improvement Activities (CPIA) and to support other MIPS requirements under MACRA. | 1.5 days |
Advanced PCMH: Mastering the Medical Home Transformation This advanced course is for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It takes a hands-on approach to the PCMH concepts and a explores the key characteristics of a successful medical home. Attendees participate in facilitated activities that demonstrate how earning elective credits builds on core criteria. What is Delegation?: Understanding the Nuts and Bolts Faculty address topics designed to help you improve your understanding of the delegation standards—from oversight to automatic credit. This interactive seminar examines key elements that should be in place before an organization gives an entity the authority to act on its behalf. It is geared to individuals who are new (less than two years experience) to managing delegation oversight, and would be especially beneficial for those involved in collecting, reporting or acting on delegation arrangements. | 1 day |
PCMH Recognition Annual Reporting: Succeeding through Continued Transformation This program is designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for submission under the redesigned PCMH Recognition program. Annual Reporting requirements will be discussed, including expectations for the transition from 2014 Recognition and for practices earning Recognition under the 2017 requirements. Faculty will explore timelines for deliverables, performance expectations and ways to demonstrate sustained Recognition through continuous quality improvement efforts and the use of data that practices collect internally (e.g., for state initiatives). | 1/2 day |
